FSGS (Focal Segmental Glomerulosclerosis) is one of the most common kidney
diseases causing renal failure in children and adults. The disease is
characterized by scarring in glomeruli; meanwhile, patients’ kidney function
declines progressively over time.
The disease attacks more commonly in kids or teenagers, and male patients are
more than female ones. Some of FSGS patients suffer from upper respiratory tract
infection or allergy before onset. The initial symptom usually presents as
Nephrotic Syndrome in clinic. Around 2/3 of those patients lives with massive
proteinuria and serious edema. About half of them suffer from blood in urine.
Mild and long-standing high blood pressure or chronic nephritic syndrome may
attack among 30%-50% of adult patients. In early and middle stage of Focal
Segmental Glomerulosclerosis, the albuminuria is usually highly selective, but
along with the progression, proteinuria leakage becomes nonselective.
As for the causes of FSGS, poisoning, humoral immunity and changes of
haemodynamics could all lead to impairment of capillary wall.
No reliable criterion of FSGS diagnosis, but rule out possible inducing
factors such as HIV infections or drug abusing etc. Asking patients’ family
history carefully and getting physical and lab examination or tests done are
helpful for the diagnosis. Kidney biopsy helps a lot in the diagnosis of Focal
Segmental Glomerulosclerosis. The disease is easily to misdiagnose as minimal
change disease, so clinical symptoms, biopsy, reaction to steroids and many
other factors should be taken into consideration to help diagnose the
disease.
There’s no effective treatment for Focal Segmental Glomerulosclerosis in
clinic. It is believed that this disease often ends up with a poor prognosis and
hard dealing with. Generally, FSGS would progress to ESRD or Renal Failure
within 5 to 10 years. In recent years, treating primary cause of Focal Segmental
Glomerulosclerosis, applying diuretics, and lowering the high blood pressure
could slow down the progression and help get a remission of Nephrotic
Syndrome.
